    Inconsistencies are flagged, and a source is requested for any statistic that will be published. A figure that cannot be confirmed is left out of the design.

    Yes. Colours, type and logo use come from your guidelines, so the graphics sit beside the rest of your content. If no guidelines exist, Logo Design and Brand Identity are the place to start.
    Two, after the first design is presented. The headline finding and the reading order are agreed before design, which is what keeps two rounds enough. Further rounds are quoted in writing.
